Reflections

The Reflections neighborhood at Van Dyk Park Place is our secure memory care unit, which provides a safe and friendly environment for our residents living with Alzheimer’s or another form of dementia.

Our Lighthouse program is designed to help residents who suffer from memory impairment and need additional assistance with socialization and community engagement.

Meet our Culinary Team at Van Dyk Park Place, led by our dietary director and executive chef, Lenz Dessources. Known for his exquisite recipes and delicious meals, Chef Lenz is truly in a class of his own. He works closely with our entire kitchen staff to embrace the utmost care and precision in providing residents with the the optimal dining experience. His philosophy as a leader is to focus on team unity, passion and fun.

Chef Lenz has a passion for seasonal food, allowing the Van Dyk family to diversify our menus throughout the year. He is a strong advocate of making many farm-to-table meals. His high expectations around presentation, taste and wellness have made a strong impression on our residents. Chef Lenz is often seen making frequent rounds across our dining room, giving our residents the feeling that they’re dining at their favorite restaurant and that they know the restaurant owner intimately.

All our chefs at Van Dyk Park Place are truly amazing. They love to put smiles on the faces of our residents, focusing heavily on recognizable recipes and comfort foods that bring back fond memories for our seniors. Our Staff

Kristin Jackson, CALA

EXECUTIVE DIRECTOR

Kristin Jackson serves as Executive Director of Van Dyk Park Place, overseeing all areas of assisted living and memory care on Van Dyk’s main campus in Hawthorne.

Kristin brings 15 years of leadership experience in senior care. She has served as an executive director in assisted living, as well as a director of operations in home care. Kristin’s leadership credentials include a strong reputation in family communication, staff development, a supportive culture, high retention of staff, high satisfaction levels among residents, grassroots community ties, and a well-connected network of physicians, nurses and hospital staff.

Kristin is family focused, maintaining best practices through personal integrity and supervising her leadership team. She continually seeks to develop new programs and activities for Assisted Daily Living, as well as Alzheimer’s and dementia practices. Kristin represents Van Dyk with its exclusive membership on the Alzheimer’s Association’s Dementia Care Provider Roundtable.

She graduated from Nova Southeastern University with a bachelor of science degree in business administration, and she is a Certified Assisted Living Administrator.

Carmen Dalton

DIRECTOR OF NURSING

As Director of Nursing at Van Dyk Park Place, Carmen works closely with our medical director and all our partnering physicians to review care plans and clinical needs, improve wellness outcomes, grow our specialty care programs, evaluate infection control measures, develop new areas for staff training, and provide the necessary education that our community deserves. She ensures that Van Dyk Park Place always has the clinical capabilities necessary to make sure that residents can age in place, and that families have access to her daily. Her work ethic, honesty, compassion and professionalism epitomize what families have come to expect of our amazing staff at Van Dyk Park Place.

Carmen joined the Van Dyk family in 2017 as Director of Nursing for At Home With Van Dyk. She played a major role in helping the agency secure advanced distinction with the Commission on Accreditation for Home Care. Under her clinical leadership, AHVD grew to unparalleled volume in 2020. Carmen managed the agency’s coordination of care and delivery of care.

Carmen began her medical career as a volunteer EMT. She worked as an LPN before eventually becoming a registered nurse. Carmen dedicated herself to a wide range of health care settings, spending 22 years in medical, surgical, ER, telemetry, mental health and geriatric care. Carmen discovered her passion for home care while working as a field nurse supervisor for a hospital, prompting her to switch exclusively to home care where she has remained for the last 12 years. She is a member of the Organization of Nurse Leaders of New Jersey. She earned her Nursing degree from Mountainside Hospital School of Nursing in Montclair.

Josie Webb

DIRECTOR OF SALES & MARKETING

Josie Webb joins the Van Dyk family with a strong background in sales, community relations and activities in senior living. In recent years, she has worked for The Chelsea at Bald Eagle where she led their activities, event planning, admissions and concierge services. This particular assisted living community was previously owned by the Van Dyk family under the leadership of Bob Van Dyk.

As director of sales and marketing at Van Dyk Park Place and a member of the company’s leadership team, Josie manages all admissions, tours, advertising and communications for Van Dyk’s award-winning assisted living community. Her energy, warmth and compassion are well known to families and health care professionals. Her commitment to exceeding expectations and helping families find solutions has made Josie a true leader in this profession. She is dedicated to cultivating strong relationships with Van Dyk’s residents, families and strategic partners.

With an extensive background in music, Josie can often be found singing with and to residents throughout the community, bringing smiles and joy through her lovely voice. In her spare time, she performs at various restaurants and bars in North Jersey, and she believes strongly in the power of music to lift spirits and infuse hope.

Josie earned a bachelor of arts degree in music from Ramapo College of New Jersey.

Alexa Green

COMMUNITY LIFESTYLE DIRECTOR

Alexa oversees all activities at Van Dyk Park Place (including Assisted Living, Lighthouse and Reflections neighborhoods) and the Van Dyk Memory Care Center. She organizes all major events for residents and families throughout the year, as well as daily and weekly Mind, Body & Soul programs (including music, dance, yoga, tai chi, cooking, art and much more). It’s no secret that Alexa and her amazing team go to great lengths to ensure that every Van Dyk resident can live life to the fullest. She loves putting smiles on our residents’ faces. And she’s just so good at it!

Alexa joined the Van Dyk family when she was a freshman at DePaul Catholic High School, volunteering during summer months and holiday seasons. She later served as an intern at Van Dyk Park Place during her junior year of college. Alexa accepted a full-time position in the activities department while completing her bachelor’s degree. She worked as an Operations Manager and later as Assistant General Manager at a gym in northern California, expanding her skills in payroll, budgeting, human resources and customer service.  But Alexa realized her passion remained in health care, specifically with seniors.

She earned her bachelor’s degree in Health Care Administration with a minor in Business Administration (and a concentration in Long-Term Care) at the University of Scranton.

Alexa’s story at Van Dyk is truly remarkable, starting as a volunteer and now serving on the leadership team at Van Dyk Park Place. Her warmth, her enthusiasm and her authenticity make our residents feel like they are truly home.

Pam Tokarczuk

Pam Tokarczuk

FAMILY LIAISON, MEMORY CARE

Pam Tokarczuk has been the top memory care expert in the Van Dyk family for 20 years. She works closely with families whose loved ones require memory care in our LightHouse and Reflections neighborhoods at Van Dyk Park Place, as well as our adult day care program.

In her decorated career, Pam has served as Executive Director of Van Dyk Park Place, Executive Director of Van Dyk Memory Care Center, Administrator of Van Dyk Manor in Ridgewood (recognized for five consecutive years by US News and World Report as a 5-star post-acute rehabilitation and skilled nursing facility), and Executive Director of Van Dyk at Bald Eagle Commons Assisted Living and Senior Apartments in West Milford, NJ. Pam has managed the quality and consistency of care services and operations, maintaining quality standards and compliance on behalf of Van Dyk Health Care.

As a Certified Dementia Practitioner, Pam is known for her compassionate care and resident advocacy. She serves on the NJ LTC Culture Change Committee, HCANJ Best Practice Committee and Convention presenter for “Changing the Culture of Dementia Care to Reduce the Inappropriate Use of Antipsychotic Medication”.

She graduated from Fairleigh Dickinson University with both bachelors and master’s degrees, and has since earned nursing home administrator license, assisted living license for the New Jersey Department of Health and Senior Services and is certified with the National Council of Dementia Practitioners.
