A Personal Approach to Care

Van Dyk Manor offers Medicare certified short-term rehabilitative and skilled nursing services, long-term care, respite and hospice services.

SHORT TERM AND SKILLED NURSING SERVICES

At Van Dyk Manor, we specialize in rehabilitative services for the older adult. Following completion of their rehabilitation program, residents continue with the restorative nursing program to maintain their strength and endurance. In addition, we offer residents Exercise Physiology in group and individual sessions. For those residents returning home, a comprehensive discharge plan will be completed by the resident's treatment team.

LONG TERM CARE

Van Dyk Manor specializes in warm, compassionate care for residents who are unable to live independently. Skilled Nursing Care is provided on a 24-hour basis utilizing an interdisciplinary team approach. The care team confers weekly to assess the needs of each resident and develop the individual care plan including: medical, nursing, social services, nutrition, activities and rehabilitation goals. The residents and their families take an active part in the process of developing the individualized care plan.

MEDICARE CERTIFIED SERVICES

Medicare Certified Services Include:

•  Physical Therapy to restore endurance and strength
•  Occupational Therapy to restore the activities of daily living
•  Speech Therapy to restore language ability and correct swallowing disorders
•  I.V. Therapy, wound care, tracheotomy care

RESPITE CARE

Families caring for seniors in their homes occasionally need to travel or take a break from the stresses care giving can cause.

Van Dyk Manor offers short-term stays that provide a secure and loving environment while their family is away.

FITNESS

Encore has recently introduced its "Senior Division". Encore's Senior Division provides exercise programs designed to help improve the lifestyle and increase or maintain physical function in older adults based on that individuals physical and mental needs. All sessions involve Encore trainers going to the nursing and assisted living homes and programs may involve walking around the room, taking longer walks outdoors when the weather is favorable, using a leg or arm bike, using exercise bands or hand weights, and the list goes on. Clients can increase walking endurance; muscular strength and endurance exercises can improve posture. Balance exercises can help to prevent falls. Flexibility exercise can increase range of motion at a given joint, even if the older adult is confined to a chair or bed. Any improvement in an older adult's physical function will result in a more comfortable and rewarding quality of life, and increased independence.  [see our Fitness Page for more information]

Garden State Parkway

1. Take the Garden State Parkway South to exit 151, Watchung Avenue. Proceed on Watchung (going West) approximately 4-5 miles. You will go through a small shopping center and a railroad crossing. Bare right under trestle (still on Watchung Avenue). You will cross Valley Road (traffic light) and go to blinking light. At blinking light turn left onto North Mountain Avenue. Proceed on North Mountain Avenue about 1/2 mile. Van Dyk Manor is on the right.

2. Take the Garden State Parkway North to exit 151. Make a left at top of ramp onto Watchung Avenue. Proceed on Watchung (going West) approximately 4-5 miles. Route 280

Take exit at 8 A or B depending which direction you are going. Take Prospect Avenue exit, Cedar Grove/Montclair. You will pass Pal's Cabin on your left. If you don't pass Pal's and you see Essex Green Shopping Center on your right, you are going the wrong way! Continue on Prospect Avenue, passing "The Manor" and Montclair Country Club on your left. At Bloomfield Avenue, turn right and continue on Bloomfield Ave. to No. Mountain Avenue (Montclair Art Museum will be on right corner). Turn left on No. Mountain Avenue and go one block, crossing Claremont Ave. After crossing Claremont Ave., our facility is second building on left. Lord Essex Garden Apartments are on corner.
