Post-Acute Care

Many conditions and treatments require short-term care after a stay in a hospital so the healing process can begin in a comfortable environment. These stays can include specialty treatments and therapies, skilled nursing care, as well as education to ease the transition back home and into your normal lifestyle. Talking to your doctor about post-acute care is important. Making decisions on your care before you are focused on your recovery can ease stress and ensure comfort with the location and philosophy of the short-term stay environment.

At Van Dyk Health Care, we create a customized care plan for you and your family, working closely with your physician, to ensure a smooth recovery and safe return to your home environment.

Wound Care

Wound Care

When you or a loved one suffers from wounds that do not heal quickly, Van Dyk can provide healing services to relieve pain, decrease swelling, increase and restore mobility, and prevent infection. Often, wound care is needed in addition to other services like vascular care, orthopedic injuries, or diabetes. With your physician’s guidance, our wound care certified staff can provide treatment for complicated wounds, including those requiring special beds and wound vac applications. And with a hospital-based wound center close by, specialty consultations are readily accessible for immediate changes to care.

Van Dyk’s Wound Therapy program treats wounds caused by:

  • Vascular disease
  • Poor circulation
  • Pressure
  • Diabetes
  • Surgery
  • Traumatic Injury

Medical Respite Care

Medical Respite Care

Van Dyk Health Care offers Medical Respite Care as part of our post-acute services for those individuals who are too ill or frail to recover from a physical illness or injury on their own but who are not ill enough to be in a hospital. Unlike “respite” for caregivers, which we offer as part of our skilled nursing care services, our Medical Respite service provides short-term residential care that allows patients the opportunity to rest in a safe and comfortable environment while accessing medical care and other supportive services until they are physically ready to return to their normal lives.

LifeMoves Orthopedic Therapy

LifeMoves Orthopedic Therapy

Injuries, accidents, or correction of failed joints all lead to the need for specialized orthopedic rehabilitation and therapy services. Extensive surgeries can prevent continuation of regular, daily activities as pain subsides and movement returns. To cope, specialized inpatient orthopedic rehabilitation is often recommended to ensure a quick, safe return to an active, independent, pain-free lifestyle. At Van Dyk, you receive specialist care as we work with your physician to develop an individualized rehabilitation and therapy plan. Specialized orthopedic rehabilitation and therapy is available for:

  • Short-term post-acute care patients
  • Long-term patients
  • Park Place residents

Planning for After-Care

When you have the advantage of planning your recovery from orthopedic surgery, you can talk to your physician prior to the procedure to determine your options for recovery. Your physician can help you determine whether you may be a candidate for inpatient short-term rehabilitation and therapy like provided at Van Dyk. Choosing early helps to ensure you are comfortable with the staff, environment, and opportunities for care beyond prescribed therapy sessions.

Inpatient Cardiac Rehabilitation Care

Recovery from a cardiac episode or surgery can require special focus from specialists who understand the stages of rehabilitation and necessary treatment of chronic conditions. Van Dyk’s HeartWays program offers patients direct care by cardiac physicians and nurses who can help you or a loved one transition back into a comfortable lifestyle at home. Healing after cardiac trauma or extended treatment of chronic heart failure requires physical and emotional care. Short-term, inpatient cardiac care is often recommended to safely monitor healing, begin infusion therapy treatment, and understand individual needs. During this recovery time, you’ll carefully be guided toward a unique, at-home care plan that includes exercise, diet, daily activity recommendations, and personal care.

Planning for Cardiac After-Care

When you have the advantage of planning your recovery from cardiac surgery, you can talk to your physician prior to the procedure to discuss your options for recovery. Your physician can help you determine whether you may be a candidate for inpatient short-term rehabilitation and therapy at Van Dyk. Choosing early helps to ensure you are comfortable with the staff, environment, and opportunities for individual healing beyond prescribed physical treatments.

Testimonials

When I left Van Dyk, a bit earlier, I believe than my experienced physical therapists might have recommended, I felt well-prepared to continue my exercises. I left to be home for Christmas. I had mixed feelings – wistfulness and deep gratitude- among them. Van Dyk had been a true sanctuary and haven for me: a gentle community of people devoted to helping and healing others. I feel blessed to be there. I thank you and your family for creating and maintaining the nurturing, warmly hospitable atmosphere of the Van Dyk and the high quality and professionalism if your staff.
