As a resident at Van Dyk Park Place, your level of assistance can match exactly what you need. From meals to housekeeping to wellness visits from a nurse, you can continue your life with the security and comfort of just the right support system. Assisted living is often the right alternative for persons desiring to continue to live on their own, but need help with certain tasks that are not medically related. Home services may help temporarily, but do not offer the security of an on-site staff available 24 hours a day, seven days a week who can help at a moment’s notice. At Van Dyk Park Place, you can enjoy your own pace in your own space with amenities, activities, and people available when you need them.

Reflections Alzheimer’s Care

At Van Dyk Health Care, we are committed to helping patients and families who struggle with Alzheimer’s disease and other memory loss afflictions. Our Reflections program is a special care, distinct Alzheimer’s unit at our Park Place Assisted Living facility.

Through this program, patients with Alzheimer’s and their families find the specialized care and comfort they need as they navigate this unfamiliar journey. We offer personal care assistance and supportive services such as housekeeping and transportation.

The advantage of Reflections is that your loved one is able to live fairly independently among his or her peers. Our trained, compassionate staff will be with you every step of the way, providing care for your loved one, as well as the support services and peace of mind for you and your family.

LightHouse Connections

LightHouse Connections was designed with the hallmark Van Dyk “We Care” philosophy.  The symbolism of the LightHouse, a structure to provide guidance, is the cornerstone of our program.  Our goal is for Memory-Impaired patients to enjoy their day in a small group environment while maintaining an independent apartment within our Assisted Living Community.

Residents experiencing Memory Loss often display the following signs:

  • Increasing memory loss and confusion
  • Shorter attention span
  • Problems recognizing friends and family
  • Repetitive statements and/or movements
  • Restlessness
  • Difficulty organizing thoughts in a logical manner
  • Problems with reading and numbers
  • Difficulty finding the right words

The foundation of LightHouse Connections is to provide supervision and support, structure and camaraderie, physical and cognitive stimulation and meaningful activities.  Staff will implement “Mind, Body, Soul” programming to encompass the resident’s well-being.

Daily lifestyle enrichment for residents allows them to continually explore, experiment and adjust.  Activities will be flexible to meet the needs of the individual and group dynamics.

Outpatient Therapy

Van Dyk Health Care at Park Place proudly offers outpatient therapy to its residents. As part of our partnership with HealthPro, our therapy begins with a thorough evaluation to identify any physical impairments and functional limitations. Our talented therapists provide personalized care, gauging progress and making adjustments to meet individual goals.

Our physical therapy is used to improve mobility and strength, restore independence, relieve pain and ensure a safe return to daily activities. Our occupational therapy is used to help residents who are recovering from an injury or illness, enabling them to regain functional abilities for daily life and independence.

We see positive clinical outcomes from our outpatient therapy, including improved self-reliance and self-determination, fewer safety risks, fewer falls, increased activity levels and improved coordination and balance.

Medicare Part B helps pay for medically necessary outpatient physical and occupational therapy. Upon receiving authorization from a physician or nurse practitioner, Medicare will pay its share until the total amount reaches the Medicare therapy cap. We accept other health insurance for our outpatient therapy, although you may be required to obtain pre-certification or pre-authorization.
