We hand-select our Certified Home Health Aides, supervised by registered professional nurses, to fit your unique care needs. We closely assess every person’s specific type of care, living environment, personality, physical and emotional needs.

As part of our home care services, our licensed staff will aid you in various activities from guided walks to shopping and religious services, so you can continue to enjoy life at your own pace. At Home With Van Dyk’s services are great for seniors who prefer to live on their own but may require a little help with some day-to-day tasks.
Personalized Care
We will create a personalized plan of care, created specifically for you and your family. Our home health aides will work under the supervision our registered professional nurses, including our Director of Nursing, to follow your personalized plan.
Weekly services may include assistance with walks, outings, errands, grocery shopping, meal preparation, medical appointments, and light housework.
Our caregivers provide the gentle touch and personal reassurance, especially when family members are unable to be there.
Transitional Care
We are always prepared to support any transitions affecting you and your loved ones – whether it’s a change in care, living environment or lifestyle. Our experienced and qualified caregivers are available to relieve the stress of handling these transitions on your own.
We will act as your advocate and steady guide to alleviate any crisis and find the right solutions to keep our loved ones at home longer.
Alzheimer’s & Dementia Care
Our caregivers are specifically trained to provide the optimum care to meet the needs of this specific diagnosis. Our goal is to provide an improved quality of life for our clients diagnosed with Alzheimer’s and Dementia.
Respite Care
As the primary caretaker, we realize that your responsibilities can be overwhelming and physically exhausting. Our Respite Care program allows families to have the peace of mind that our experienced and qualified caregivers are always available – helping their loved ones whenever a primary caretaker needs a break in their day.

Christine Sanchez
Executive Director, AHVD
Vice President of Home Care Solutions, VDHC
As executive director of At Home With Van Dyk, Christine Sanchez oversees all areas of customer service, business development, marketing, staffing, strategic partnerships and regulatory compliance on behalf of the agency. She has served in a leadership role in home care since 2010, previously as a Regional Director. Christine believes strongly in building talented leaders within an organization — a core value of the Van Dyk family for decades. Her sincere desire to make a positive impact on families and colleagues is felt at all times. Christine earned a bachelor of arts degree in communication from Rutgers University.
Julie Cowan
Director of Nursing
Julie brings over 20 years of nursing experience to the Van Dyk family. Her warmth and compassion make a profound impact in the lives of the many seniors we proudly serve. She has a proven track record of building strong clinical teams to serve in home health settings. As Director of Nursing, she is the chief clinical leader of Van Dyk’s home care division, overseeing the company’s coordination of care and delivery of care. She helps maintain compliance with all regulatory commissions. Julie started her nursing career as a field supervisor for a home care agency in Florida and then later become a director of nursing in New Jersey. She earned her BSN degree from South University.
Denise Meeks
Assistant Administrator
Denise joined the Van Dyk family with over 25 years of experience in business management and office operations. She has been a member of the AHVD leadership team since its opening. A consummate professional with vast credentials in healthcare, Denise provides a steady presence in the AHVD office with her strong analytical, problem-solving and people skills. Her strong background in finance and accounting has allowed her to manage the company’s billing and collections, accounts payable, accounts receivable, payroll processing, and financial reporting. She previously served as a Business Manager for a mid-sized consulting firm, where she efficiently handled human resources, accounting and client relations. Denise began working in home care in 2012 and continues to shine in a profession that she loves dearly.
Elina Polyakov
Director of Concierge Services
Elina joins the Van Dyk family with over 12 years of experience providing social work and concierge services to seniors living at home. Her primary focus is on customer satisfaction and her motto is to do “whatever it takes” to ensure the clients are well cared for and happy while preserving their dignity and independence. Elina provides regular home visits, performs assessments, advises on care plans, monitors care, offers crisis intervention and supportive counseling, and linkages to other professionals. Elina also serves as a support to the caregivers and advocates their needs and concerns to either the client, family and/or the Company. Elina holds a master’s degree in social work from Columbia University School of Social Work and has a strong background in psychotherapy bringing these much-needed skills to her current role. She is passionate about her profession, and she treasures every moment and every interaction she has with her clients and their families, learning in the process about resilience and overcoming challenges of aging.
Carolann Townsend
Director of Human Resources & Compliance
Carolann brings over 14 years of home care experience to the Van Dyk family. Starting her career with the home care division of a top medical center in Northern New Jersey, she was the liaison between the home care division, the medical center and physician offices. Carolann’s compassion and outstanding customer service with clients, families and caregivers is reflected in her providing quality care into our clients’ homes. As Director of Human Resources and Compliance, Carolann oversees all aspects of staffing, recruiting, training, regulatory affairs and technology integration for scheduling.
Ashley Dohm
Client Care Specialist
Ashley brings a stellar record of customer service to the Van Dyk family and everyone we serve. As a client care specialist, Ashely’s primary duties include coordinating schedules with our certified Home Health Aides to ensure we are able to match aides based on specific client needs. She is a powerful force in the workplace, relying on her positive attitude and tireless energy to encourage others to work hard and succeed. Ashley graduated from William Paterson University in 2019 with a Bachelor’s degree in Criminal Justice and a minor in Sociology.
